General Information on Reports
Any report indicating non-conforming materials or work, shall be submitted immediately. All other reports must be submitted within 14 days of the date of the materials test or special inspection. Reports shall be submitted to the following entities:
- The school district
- DSA
- The architect or engineer in charge of the project
- The structural engineer delegated responsibility for
structural aspects of the project (when structural work is delegated), and
- The project inspector
Test Report Templates
Test and special inspection report templates are provided for the convenience of LEA accepted testing facilities. Reports conforming to these formats must be submitted for projects under DSA jurisdiction. Note that test reports need not match these templates exactly. Additional information may be provided, information may be rearranged, or other minor changes may be made. However, use of these standardized templates is strongly encouraged since standardized forms will expedite review of reports at DSA and increase the probability that reports will be acceptable to DSA.
