Options and Instructions for Completing the Record Set Handling Form (DSA-145)

Options: The client has three options to fill out the DSA-145 form.

1. With a computer, the client can go to the DSA-145 form on the DSA web site and fill out the form electronically. The client then prints the form and brings it to DSA with his or her Original Documents. The DSA-145 form must be signed by the client prior to the drawings being stamped.
   
   Note: The client's computer must have Adobe Reader 7.05 or later to view the DSA-145 form. Adobe Reader is free and can be downloaded from http://www.adobe.com/.
   
   
2. The client can come to DSA with the Original Documents and use the computer in the backcheck room. The client just opens the DSA-145 application and fills out the DSA-145 form electronically. The client then prints the DSA-145 form and signs it prior to the Original Documents being stamped.
   
   
3. The client can come to DSA with their Original Documents and fill out the DSA-145 form manually and sign it. The DSA-145 form must be signed prior to the Original Documents being stamped.

Instructions: Form DSA-145 is fillable on line. This means you can enter requested information directly into the DSA-145 form on screen.

Sample forms: The examples that follow show the DSA-145 forms filled in with fictional data to illustrate a properly filled in form for:

• Scenario #1 - Client requests Original Documents be returned using a delivery service.
• Scenario #2 - Client requests Original Document be held for pickup.
